What You Will Do to Change Lives

The Clinician provides mental health support services to members served and their families at the program. The Clinician also assists in the coordination of all mental health support services provided by the Team. This includes coordinating and collaborating with team members and outside agencies such as, the Conservator's Office, Public Guardian's Office, Probation Department, County Behavioral Health Services, and housing and inpatient providers.

Shift Available:

Full Time Salaried; Monday - Friday; 8:00am - 4:30pm

Expected starting wage range is $68,033.91 - $84,024.91. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

  • A Master's Degree in Social Work or related field.
  • Registered with the CA BBS with a ASW, AMFT, APCC
  • Two (2) years of experience post masters in mental health setting
  • Personal car that you are willing to use for work
  • Willingness to transport clients in your personal car
  • Driver's License that is valid and in good standing

Program Type: Community-based

To Make Referrals: Telecare Gateway to Recovery ACT is a voluntary closed-referral program that assist adults and Transition Age Youth (TAY) in their recovery as they transition from long term locked treatment facilities. They also serve persons who are identified on San Diego County's current High Utilizer List.

# of Members: 235 members

Who We Serve:

  • Must be a resident of San Diego County
  • Adults and Transition Aged Youth(TAY) with Medi-Cal
  • Must have a qualifying Title IX diagnosis and may also have co-occurring substance use diagnosis
  • A county referral for persons stepping down from a long-term care facility or a person on San Diego County's high utilizer list
  • Long-term care facility diversionary referrals may be considered at the County Monitor's discretion
