Community Health Worker

Job Description

The School of Medicine Pediatric Hospitalist team is seeking Community Health Worker to provide essential support and health navigation to immigrant survivors of torture and trafficking, assisting them in accessing necessary medical services and resources. This role involves the development and maintenance of a robust referral system to connect clients with primary care and other medical services, ensuring comprehensive support. A key aspect of the role is building meaningful relationships with clients to understand their challenges within the healthcare system and to provide holistic and sustainable resources. The Community Health Advocate will engage directly with Asylee Women Enterprise case managers/staff, clients, and HEAL clinicians to offer comprehensive primary care support, in addition to providing support to HEAL’s mental health programs for survivors of trafficking where needed.


Specific Duties & Responsibilities

  • Ensure confidentiality and secure handling of all participant information.
  • Maintain accurate records of individuals and families enrolled in the primary care services.
  • Directly engage with Asylee Women Enterprise Case Managers, clients, and HEAL clinicians to provide comprehensive primary care support.
  • Attend essential HEAL staff meetings as well as designated staff meetings for Asylee Women Enterprise.
  • Provide interpretation ad hoc for clients.
  • Develop meaningful relationships with clients to better understand their struggles within the healthcare system and find holistic and sustainable resources.
  • Assist in implementation and maintenance of pharmacy delivery program of prescription medications for HEAL Primary Care patients.
  • Provide as-needed support for Forensic Evaluation Clinic evaluations and logistics.
  • Support quality-improvement projects from a client-based perspective.
  • Maintain and expand referral system to connect HEAL Primary Care patients with other medical services and resources.
  • Assist in implementation and curriculum-building for the Steps to Success Spanish-language peer support groups.
  • Assist in the development of Behavioral Health Integration infrastructure for the SOT Primary Care program.
  • Provide health navigation and support to immigrant survivor of torture.


Special Knowledge, Skills, & Abilities

  • Spanish and/or French language fluency is preferred.
  • Must be comfortable working with the participant population. Experience working with uninsured low-income immigrants with limited English proficiency.
  • Ability to judge when to seek supervision.
  • Flexible hours required.
  • Knowledge and understanding of Federal statutes regulating confidentiality of drug and alcohol records and laws for reporting of child and elder abuse is required.
  • Experience with computers including word processing, databases and Excel spreadsheets.
  • Strong understanding of the healthcare system and services available to asylum seekers and immigrant populations.
  • Knowledge of rights and resources for asylum seekers and survivors of torture.
  • Ability to provide interpretation services in Spanish on an ad hoc basis.
  • Comfort with clinical conversations and concepts. Ideally, the applicant will have prior experience working in a clinical setting.


Minimum Qualifications
  • High School Diploma or graduation equivalent.
  • Two years related experience.
  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.


Preferred Qualifications

  • Spanish and/or French language fluency is strongly preferred.
  • Background in mental health or prior mental health-focused experience.
  • Prior training in trauma-informed care/practice and commitment to a trauma-informed approach.
