Job Roles

  • Build positive relationships, inspire others to perform at a higher level, prepare career development plans and retain an accountable workforce.
  • Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
  • Participate in continuous improvement activities and ensure appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
  • Provide/ensure staff receive all new hire and annual trainings on time.
  • Integrate social workers in the medical clinic setting to support patients, their families, and the interdisciplinary care team.
  • Develop electronic resource library and promote linkage of all social services throughout the company; either directly, or as a consultative service line coordination.
  • Oversee expansion of screening methods for SDOH, SOGI, PHQs, and other assessments that lead to the connection of care management services.
  • Develop and deliver managed care business strategy through innovative approaches and directives.
  • Ensure continued development of FHCSD episodic and longitudinal care management services to include expansion of the Health Homes Program and enhanced care management.
  • Responsible for developing, standardizing, and maintaining Care Management staff competencies and job skill assessments.
  • Foster and sustain FHCSD's community hospital relations to maximize referrals and promote positive patient care transitions.
  • Develop hospital-based social services to increase patient transitions to primary care and mental health services at FHCSD.
  • Work effectively with IT's Business Intelligence team to develop strong performance metrics and measures of impact adjusting strategies based on outcome data.
  • Collaborate with clinical leadership to coordinate services and develop strategies for risk mitigation and healthier patient outcomes.
  • Work with CEO and CFO to ensure financial stability of department and services.
  • Perform other duties as assigned.

Education/Certifications/Licenses/Registrations

  • Master's degree in one of the following disciplines: Public Health, Business, Health Care, Nursing, or other relevant fields. OR Bachelor's degree in a relevant field requiring 5 to 7 years of experience in senior health care management.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.

Experience/Specialized skills (including Language)

  • 3 to 5 years of experience in program management.
  • 5 to 7 years of progressive experience in senior health care management.
  • Action-oriented to include sound negotiating skills relative to responsibility held.
  • Demonstrated ability in maintaining good interpersonal interactions with employees, coworkers, and departments.
  • Strong analytical and problem-solving skills to include critical thinking skills.
  • Strong verbal and written communication skills with the ability to positively interact with a broad and diverse range of cultures, socio-economic groups, and personnel.
  • Knowledge of supervisory practice and techniques to include performance management and employee development.
  • Successful track record of leading and driving positive change through others.
  • Knowledge of and ability to create, maintain, and adhere to budgets, fiscal planning, grant programs, and reporting requirements at a managerial level of responsibility.
  • Bilingual in English/Spanish preferred.
  • Possess exceptional organizational skills with attention to detail that include time management skills, with a strong motivation in meeting deadlines and achieving established goals.
  • Knowledge of community resources, health, and social services systems in San Diego County with a willingness to work within these community settings appropriately.
  • Intermediate skills in computer applications and software to include MS Office applications, and Electronic Medical Records.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$120,000.00 - $170,000.00

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
